This webinar will be hosted bySian Block, NAPC Faculty Member, and will discuss ‘What is Care Navigation?’, help your understanding of Care Navigation, and the effects that positive care navigation can have with patients and all primary care staff.

The webinar will focus on:
•The role of the primary care team, roles and responsibilities, services, and the role of care navigators as part of the multidisciplinary team
•Building a community of practice across Practices and or PCNs for continued learning and development
•How attributes and skills are important for care navigation including emotional intelligence for care navigation, skills development and practise
•Building rapport, listening, asking open discovery questions, problem solving and managing difficult conversations
•Understand the impact care navigation can make
