Please ensure you complete the below form in a timely manner each month and please note you can only record 1 months mentoring at a time.
- STEP 1 – Please ensure you select your name and reporting month from the drop down.
- STEP 2 – Entering your unique password in the relevant field will then generate a list of your current mentees. (passwords are case sensitive)
- STEP 3 – Please ensure you select “yes”, “no” or “paused” for each mentee and include any relevant information in the notes box.
- If a mentee has now completed thier allocated mentoring, please select paused and include the date of their last session in the notes box.
- If a mentee is not listed please contact email@example.com
- STEP 4 – Please indicate if you have capacity for additional mentees
- STEP 5 – Please select the “i agree” box at the bottom and then submit the form.
