We would like to invite you to participate in the CARE Programme.
As set out in the NHS Long Term Plan, local NHS organisations will increasingly focus on population health and local partnerships with local authority-funded services, through Integrated Care Systems. The CARE Programme will enable you to connect with like-minded people to develop and effect health changes locally.
This is an excellent opportunity for networking across Primary Care and Community roles, gaining insight into your local system and future of the South West region

The CARE programme is delivered by the National Association of Primary CARE (NAPC), consisting of 8 x 2-hour sessions run weekly, with a focus on both population health management and wellbeing and resilience. An overview of the sessions can be found attached, with delivery scheduled as follows:
